2) more importantly, if there was a free, readily-available vaccine which would reduce the harmful effects and the smoker, alcoholic, drunk driver refused to take it... yes, indeed, I believe they should also be refused medical care as well.
There is, it's called not smoking, not drinking, and not driving drunk.
